We have several programs that provide family support. We will help you find the right one for you.
Family Wellness Program
The Family Wellness program helps pregnant and parenting families adjust to their roles as parents. You and your nurse will work together to decide how often and how long you want to use this program.
Healthy Families America (HFA)
The Healthy Families America Program helps parents give their baby the best start possible. A Public Health Nurse will visit your home in your baby’s early years. Your nurse will help you find ways to raise a healthy, happy baby. Visits in this program are done weekly at first, and then less often as your family’s needs change.
Nurse Family Partnership (NFP)
Nurse Family Partnership is a program for first-time moms. As a first-time mom, you most likely have a ton of questions. That's why Nurse Family Partnership starts working with you before you give birth to your child. Your nurse will visit you every week or two during your pregnancy and up until your baby is two years old.
Teen Parent Program
The Teen Parent Program is for pregnant and parenting families under 19 years old. A Public Health Nurse will help guide you through your role as a parent. Nurses can also help you with the balance you will need as a parent and student if you are in school. The nurse will also help you set and reach goals for yourself and your child.
